Long Car Rides and Back Pain: What Helps Before, During and After the Drive

2026-09-28 6 min read · Reviewed by Dr. Gregory Martin

Long car rides are a common trigger for back pain, and the good news is that much of the risk comes from how the seat is set up and how you treat the hours in it, which means you can do something about it before the trip even starts. The goal is simple: keep your spine in a supported, neutral position and break up long stretches of stillness.

Start with the seat. Sit with your back against the seat rather than leaning toward the wheel, and adjust the seat so you can reach the pedals without stretching. A small pillow, rolled-up towel or lumbar cushion between your lower back and the seat can help maintain the spine's natural curve, since most car seats offer little lower back support. Keep your knees roughly level with or slightly higher than your hips, and empty your back pockets: sitting on a wallet or phone can tilt the pelvis and add strain.

During the drive, take breaks, and take them often. Getting out to walk and stretch every couple of hours lets the spine unload after being held in one position, and drivers who plan stops tend to arrive less stiff. If you are the passenger, the same rules apply, and you also have more freedom to change position through the trip.

After a long drive, gentle movement helps more than flopping on the couch. A short walk, some easy back and hip stretches, and a hot shower can ease the stiffness that builds up from hours in one spot. If your back complains every time you drive, the pattern is worth understanding rather than just rotating which stretches you try.

For drivers who already deal with back pain, Cleveland Clinic recommends talking through driving adaptations before a long trip, and it is reasonable to plan shorter stints behind the wheel. When stiffness and discomfort from driving keep showing up, an evaluation can help pin down why. Dr. Gregory Martin has practiced chiropractic since 1996 and sees patients in Sparta, New Jersey, and Morris Plains, New Jersey. His approach starts by listening to your history and looking at how your spine actually moves, then telling you honestly what he finds.

Fall is a popular time for weekend drives to the lakes, trails and lookouts around Sussex County, and a few adjustments can make the trip far easier on your back. If driving has become a reliable source of back trouble, that is worth having looked at close to home. Call or text (973) 699-4100 to schedule an evaluation.
