Sciatica vs. Piriformis Syndrome: What's the Difference?
2026-10-02 6 min read · Reviewed by Dr. Gregory Martin
Sciatica is a symptom, not a single condition. It describes pain that travels along the sciatic nerve from the lower back, through the buttock and down the back of the leg. Piriformis syndrome is a specific possible cause of sciatica-like pain, in which the piriformis muscle in the deep buttock tightens or spasms and presses on the sciatic nerve. The difference matters because the two usually point to different parts of the body and different evaluations.
What sciatica usually means. Sciatic pain typically starts in the lower back and follows a fairly predictable path down the leg, often below the knee. It can come with numbness, tingling or weakness, and it is usually related to the lower spine: a herniated disc, narrowing, or a bone spur irritating a nerve root. That is why any evaluation of leg pain that travels from the back has to include the back itself.
What piriformis syndrome looks like. Piriformis-related pain tends to sit deep in the buttock or hip, and it is often worse with prolonged sitting, with stairs, and with certain hip movements, along with tenderness over the buttock area. It can radiate down the leg but usually stays above the knee. Diagnosis is often a process of ruling out other causes, because no single test proves it.
Why the difference matters. Care for sciatica most often starts with the lower back and pelvis, while care for piriformis syndrome starts with the muscle and hip. A useful evaluation looks at both: how your spine moves, how your hip and pelvis move, and exactly what makes the pain worse. When Dr. Gregory Martin evaluates this pattern, he takes that whole-picture approach, and if the findings point toward a medical condition rather than chiropractic care, he will say so plainly.
Red flags. Loss of bladder or bowel control, numbness in the groin area, progressive weakness in a leg, or symptoms that started after a fall or accident need prompt medical attention. Pain that stays severe, keeps worsening, or lasts more than about a week is worth a medical evaluation first.
